Social Worker – Integrated Services
This is an exciting opportunity for an experienced social worker to join our North Monmouthshire Integrated Team. We work in and around Abergavenny and the wider Monmouthshire area to support people with a range of impairments to live their lives. We work in local hospitals to support people who need help with their discharge planning. We welcome applications from enthusiastic social workers who want to work full-time or part-time. This role is focusing on hospital discharge. We are looking for an experienced social worker who is able to work with a good degree of pace, and is able to work together with our colleagues from health, to ensure a safe and successful discharge from hospital. The ideal candidate will have experience with hospital discharge and is able to work autonomously, as well as part of a team.
