Wellbeing and Support for Adults
We support people to stay independent and maintain choice and control over their lives. We will work with you to identify what matters to you, looking with you at your choices among the range of options that are available. The aim is to support you to maintain a good quality of life, or help you find other people who might be able to help.
The support we provide may come in the form of creating better links between people and their communities or beginning an assessment that will determine whether there is a need for more formal support.
In Wales, a law was passed that aims to maintain and improve the wellbeing of people in need. This is known as the Social Services and Wellbeing (Wales) Act 2014. The way we work is guided by this law. We aim to:
- Listen to understand what is important to you
- Communicate clearly with you
- Work with you and bring together only those people who can help you to live your own life
- Be sensitive to where we listen to you
- Work with you to find solutions to any difficulties you might be experiencing
- Actively engage with our local communities to develop opportunities to help people to strengthen their support networks
- Use our resources as creatively and flexibly as possible.
Who might want to get in touch with us?
If you are an adult (over 18 years) who has:
- A short or long term health condition
- A physical disability
- A learning disability
- Mental health needs
- An autistic spectrum condition
- Or if you are a carer
We will work with you to understand what matters to you, and whether you are eligible for social care support. If you qualify for support, you will have a financial assessment to see if you need to pay toward the cost.
